If you have need of a replacement or duplicate award statue from any year of the IDMA or DVDA Excellence awards,
(and are duly authorized to receive it), just fill out this form:

1 - Fill out the form below (fields with * require completion)

This form is meant to provide information for purchasing a replacement or duplicate(s) of ONE award. If you need a replacement or duplicate for more than one Winning Award (i..e. you won for three different awards, and would like copies of all three for your company wall of fame) please submit a form for each award you would like. Your web Browser should remember the common information, like your name, address, etc., so entering it again will not be time-consuming.

2 - Click the Submit Request button to submit the form

 

Your request form will be sent to IDMA by email,  and a copy sent to you by email.
You will see a confirmation page with PayPal button;

 

3 - Click the PayPal button on the confirmation page, or visit the IDMA store on the IDMADVDA.ORG site to pay for the award(s)

That's all there is to it!
Your email from us confirms your request, and your email from PayPal will confirm payment.
f we need any further information, we will contact you.
In the event we cannot honor the request, your payment will be refunded.
PLEASE READ CAREFULLY:

Each DVDA Award is hand-made to order; because of this:

  • An award cannot be ordered without complete and accurate engraving instructions -
    It is YOUR responsibility to provide this information to us in an accurate form.
    We cannot take responsibility for this other than passing it along intact.
  • Awards take a few weeks to manufacture and shipping
  • Shipping cost (FOB Seattle, WA) is your responsibility -
    we will advise you the amount due once we receive the bill.
  • We cannot order a replacement award without verifying the award.
  • We cannot process an award request without payment. You can pay in advance at the DVDA Store, or provide payment via PayPal at the time you place your order.
